Serve Elk and Cameron County residents that have been or are at risk of being incarcerated, admitted to the LTSR, and/or under the supervision of probation/parole by ensuring that services will be consistent with the philosophy of Recovery Model of community based mental health care.

DESCRIPTION OF WORK Participates in Interagency/Multi-disciplinary team meetings, attends state and regional meetings when applicable. Develops with the individual person-centered plans focusing on meeting the individual's needs relating to housing, behavioral health treatment, substance abuse treatment, medical, social and legal issues. Collaborates and consults regarding incarcerated individuals with the Forensic Team to determine eligibility for Forensic In-Jail Behavioral Health Services, while offering provider choice. Prepares and maintains accurate and timely case notes. Maintains/updates/revises individual plans as deemed necessary.

Completes referrals to Community Health, Behavioral Health services, as well as other agencies as identified in the individualized plan/re-entry plan. Provides case management services up until the time one is identified and assigned in the community. Works with Program Specialist II in identifying gaps in services and resources for this population. Provides crisis intervention/support when necessary.

Develops individual re-entry plans in coordination with the criminal justice system. Serves as primary contact for LTSR, local and state prisons, as well as Torrance State Hospital Forensic Unit. Assists with transportation needs of individuals being released from local prison/LTSR as needed. Advocates for and notifies ID System of any individuals with developmental disabilities being incarcerated or at risk of incarceration.

Work Schedule and Additional Information:

Full-time employment. Work hours are: Monday - Friday, 8:00 am to 3:30 pm, with a 30-minute lunch or Tuesday - Friday 7:00 - 4:15 with a 30-minute lunch. Very robust PTO and Medical Benefits offered! Check your email, including spam/junk folders, for these notices.   REQUIRED EXPERIENCE, TRAINING & ELIGIBILITY QUALIFICATIONS Minimum Experience and Training Requirements:

Six months of experience as a County Caseworker 1; or Successful completion of the County Social Casework Intern program; or A bachelor's degree with a social welfare major; or A bachelor's degree which includes or is supplemented by 12 college credits in sociology, social welfare, psychology, gerontology, criminal justice, or other related social sciences and one year of professional social casework experience in a public or private social service agency; or Any equivalent combination of experience and training which includes 12 college credits in sociology, social welfare, psychology, gerontology, criminal justice, or other related social sciences. Applicants will be considered to have met the educational requirements once they are within two months of graduating with a qualifying degree/credits. Other Requirements: PA residency requirement is currently waived for this title. Hiring preference for this vacancy will be given to candidates who live within Elk or Cameron County. If no eligible candidates who live within Elk or Cameron County apply for this position, candidates who reside in other counties may be considered. You must be able to perform essential job functions. Legal Requirements: This position falls under the provisions of the Child Protective Services Law. Under the Law, a conditional offer of employment will require submission and approval of satisfactory criminal history reports including, but not limited to, PA State Police clearance, PA Child Abuse history clearance, and FBI Fingerprint clearance.
