Description The Clinical Care Coordinator, in alignment with the goals of PCMH (Patient Centered Medical Home),will be primarily responsible for panel management within primary care. The Clinical Care Coordinator is responsible for establishing and maintaining multiple working relationships with; providers, internal and external care teams, front office assistants and primary care core leaders to ensure optimal quality health care. Required qualifications: Graduation of a formal Medical Assistant program. upon hire: Washington Medical Assistant Certification. upon hire: National Provider BLS - American Heart Association. 2 years Healthcare experience.
