Homeless Case Manager

  • Monday -- Friday, 8:30 AM -- 4:30 PM

The Homeless Case Manager will be responsible for working with homeless families and individuals to identify problems, help them set goals and plan to meet those goals. Also responsible for service delivery, information, referral, monthly follow-up and completing annual assessments, as well as additional responsibilities.

What you'll be doing?

  • Establish linkages with programs, and refer clients to services provided by outside agencies, follow-up to assure receipt of services.
  • Determine program applicant's eligibility for each program; income, disability, veteran, and need for project's participation, through a comprehensive household need assessment.
  • In conjunction with the client, identify major problem areas, develop a goal plan to impact those problems, and establish a time frame to meet goals.
  • Refer clients to all appropriate human services and job training opportunities; follow-up to assure receipt of services.
  • Work closely with Staff, Housing Liaisons, PHD/NCCDC Counselor, and ESG Case Manager to ascertain coordination of services.
  • Provide services needed by case management clients that are not otherwise available, including, but not limited to, life skills education, budget counseling and transportation.
  • Maintain complete and accurate case files; entering intake information case notes, goal plans participants' progress, etc. through the agency client database, and HMIS in a timely fashion.
  • Conduct as needed one-on-one interviews with program participants via phone or in person to follow-up on goals, referrals, and other needs.
  • Maintain strict confidentiality; obtain releases and work closely with the community contacts (Case Managers, Agencies, Landlords, family members, etc.)
  • Maintain accurate and complete records, and reports.
  • Work as a team and maintain a positive attitude.
  • Other duties as assigned.

Candidates must meet the following requirements for consideration:

  • Self-motivation to complete daily tasks and procedures without close supervision.
  • Ability to ensure the integrity of data entered.
  • Willingness to work with team members to achieve desired outputs
  • Knowledge of the respective county, employment is in Indiana, and familiarity with the social service network and community resources.
  • Demonstrated ability to communicate effectively with all members of the community.
  • Working knowledge of computers, including MS Word, Excel, and be willing to learn and utilize agency software programs such as ORS, as they pertain to this position.
  • Good organizational skills and have the ability to meet deadlines in a timely manner.
  • Must have a minimum of one year of case management experience and/or training in related field.
  • Must have a valid PA Driver's license, reliable transportation and be willing to travel in and out of Indiana County if required. Also must be willing to transport Clients using personal vehicle.
  • A Motor Vehicle Report and Act 33 & 34 clearances and FBI clearance with fingerprinting obtained upon hire.
  • Must have vehicle insurance to comply with agency standards of coverage.
