SCL Health Obstetrical Care Navigator in Billings, Montana

Job Description:

The Obstetrical Care Navigator collaborates with the multidisciplinary team in the ambulatory care setting, working directly with patients to support the patient's care plan in order to help assist them with social determinates of health, behavioral health and substance use issues that might be affecting their pregnancy and improve their overall health outcomes.

ESSENTIAL DUTIES AND RESPONSIBILITIES

1. Coordinates the care and services for patients identified as needing assistance or meeting Care Management criteria based on screening tools and provider and nursing referrals. Collects in-depth information about a patient's medical, functional, and social condition, to identify individual needs in order to develop a plan to meet those needs. Works with patient and family/caregivers to determine specific goals and actions based on assessment.

2. Collaborates with the multidisciplinary team to support the patient's plan of care, including but not limited to, promoting self-management activities, assuring access to services, providing individualized support and reinforcement of education, determining the need for continued services, and connecting patients and families with available community resources, as needed.

3. Enhances the quality of patient care through effective and efficient use of resources.

4. Participates in patient appointments with providers and community resources, as needed.

5. Serves as support for patient self-management. Interviews patients and families to assess current strategies for self-care and available support systems to manage health needs.

6. Builds effective working relationships with patients, in order to assist navigation of the healthcare system and community resources.

7. Work effectively with community partners to identify opportunities and to support patients' access to appropriate resources.

8. Attends, facilitates and participates in rounds and case conferences. Advocate for patient rights. Monitors efficiency and availability of services and evaluates outcomes through variance tracking, data collection

9. Identifies needs, facilitates, or provides education to physicians, RN's and other and ancillary staff regarding case management, community resources, and roles. Participates in multidisciplinary groups and development of guidelines.

10. Conducts phone calls, providing care coordination and support, including patient/caregiver/family education in the patient's plan of care, while addressing the social determinants of health and other barriers to optimal health outcomes for the patient and their baby.

11. Promotes mission, vision, and values of Intermountain Health, and abides by service behavior standards.

12. Performs other duties as assigned.

*This is an ambulatory position, not inpatient*

Required:

Bachelor's or Master's Degree in Social Work from an accredited institution

Experience Required:

Demonstrated experience in healthcare or social services experience in an acute, post-acute, or ambulatory care setting with a focus on women and children.

Preferred:

Demonstrated experience with various populations, including those with limited financial resources, limited education background and communication barriers.

KNOWLEDGE, SKILLS, AND ABILITIES

1. Demonstrated exceptional interpersonal and relationship building skills required to effectively

collaborate with multiple stakeholders and community partners.

2. Demonstrated experience using Desktop software including Microsoft applications.

3. Demonstrated ability to navigate an Electronic Health Record

4. Knowledge of Community, Social, Health, and Welfare resources.

5. Knowledge of Legal and Regulatory issues and resources for complex patient populations.

6. Demonstrated critical thinking for problem solving and prioritization skills.

8. Demonstrated experience in program management, organizational and customer service skills

The hourly range for this position is listed below. Actual hourly rate dependent upon experience.

$26.00 - $43.54
